How to find the club sport bengaged registration form:

1. Log into your Bengaged with your BU login

2. On the left, scroll down on the task bar to My Surveys and Forms

3. In the search bar, search club sport participant registration form

4. If you need to update information at any point throughout the semester (adding/removing driver forms most likely), simply find the form in this way, click edit on your form, and update your information
